目的 觀察阿奇霉素聯(lián)合陰道抑菌凝膠治療女性生殖道解脲支原體感染的臨床療效。方法 選擇牡丹江醫(yī)學(xué)院附屬第二醫(yī)院婦科門診生殖道解脲支原體感染者120例，隨機分為治療組（70例）和對照組（50例），對照組患者第一天服用阿奇霉素1 g/次，1 次/d，中間不用藥，3 d后0.5 g/次，1 次/d，連服5 d，共治療9 d。治療組患者每晚睡前清洗外陰后，將陰道抑菌凝膠推入陰道深部，每晚1支，連用9 d（如遇經(jīng)期停用），阿奇霉素的用法及用量同對照組；兩組患者停藥5 d后來醫(yī)院復(fù)查，做解脲支原體培養(yǎng)，結(jié)合臨床癥狀、體征判斷療效。結(jié)果 治療后，治療組和對照組的顯效率分別為84.3%、54.0%，兩組比較差異具有顯著性（P＜0.05）。治療組中有2例（2.86%）患者陰道上藥后出現(xiàn)陰道灼熱感，外陰皮膚瘙癢，停藥后消失；對照組3例（6%）服藥后出現(xiàn)腹瀉、惡心等胃腸道反應(yīng)，停藥后自然好轉(zhuǎn)。結(jié)論 阿奇霉素聯(lián)合陰道抑菌凝膠治療女性生殖道解脲支原體感染具有較好療效，且使用方便，不良反應(yīng)少，值得臨床推廣。

Objective To observe the effect of azithromycin combined with vaginal antibacterial gel in the treatment of female genital tract mycoplasma infection in clinic. Methods Patients (120 cases) who were infected with genital tract mycoplasma in Out-patient Gynecological Department, Second Affiliated Hospital of Mudanjiang Medical College, were randomly divided into the treatment group (70 cases) and control group (50 cases). Patients in the control group were taken azithromycin 1 g once daily for the first 1－3 d, after 3 d, 0.5 g/d, once daily for 5 d, a total of 9 d. Patients in the treatment group washed the vulva before sleeping every night, then pushed vagina antibacterial gel into the vagina, once one night, for 9 d (menstrual disabled). Usage and dosage were the same with the control group. Two groups of patients came back to the hospital when the drug was stopped 5 d later, and they did ureaplasma urealyticum. Efficacy was determined by culture results combined with clinical symptoms and signs. Results After the treatment, the significant efficiency of the treatment and control groups was 84.3% and 54.0% with significant difference (P < 0.05). Two cases (2.86%) in the treatment group appeared vaginal burning and genital itching, and those symptoms disappeared after stopping drugs . Three cases (6%) in the control group appeared diarrhea, nausea and other gastrointestinal reactions, which turned better after stopping drug. Conclusion Azithromycin combined with vaginal antibacterial gel in the treatment of female genital tract mycoplasma infection has a better curative effect, and the usage is convenient with less adverse reaction, which is worth the clinical promotion.
