目的 比較亮丙瑞林和曲普瑞林治療子宮內(nèi)膜異位癥（EMs）的近期不良反應(yīng)與激素水平的下降情況。方法 將80例卵巢子宮內(nèi)膜異位囊腫剝除術(shù)后的患者隨機(jī)分成2組，進(jìn)行前瞻性臨床觀察。術(shù)后亮丙瑞林組注射亮丙瑞林3.75 mg，曲普瑞林組注射曲普瑞林3.75 mg，兩組均治療3～6個(gè)月，每28～30 天 1次。觀察2組的近期不良反應(yīng)，同時(shí)測(cè)定用藥第0、7、21 天血清卵泡刺激素（FSH）、促黃體激素（LH）和雌二醇（E2）水平。結(jié)果 亮丙瑞林組27例與曲普瑞林組33例全程完成了治療。兩組患者均可發(fā)生潮熱、盜汗，痤瘡，肌肉、關(guān)節(jié)痛，頭痛，異常陰道出血等不良反應(yīng)，痤瘡、頭痛的發(fā)生率兩組有顯著性差異（P＜0.01，0.05）。兩組患者用藥前基礎(chǔ)性激素水平差異無統(tǒng)計(jì)學(xué)意義；用藥治療7 d后血清FSH、LH、E2水平差異均有統(tǒng)計(jì)學(xué)意義（P＜0.05，0.01）；用藥治療21 d后血清FSH、E2水平差異均有統(tǒng)計(jì)學(xué)意義（P＜0.01）。結(jié)論 亮丙瑞林與曲普瑞林治療子宮內(nèi)膜異位癥后其激素水平變化和近期不良反應(yīng)均不同。亮丙瑞林治療后激素水平下降緩和，不良反應(yīng)發(fā)生率低。臨床上應(yīng)該根據(jù)不同劑型的特點(diǎn)，實(shí)施個(gè)體化治療。

Objective To compare the side effects and decrease of hormone level in the treatment of endometriosis (EMs) using Triptorelin and Leuprorelin. Methods Ovarian endometriosis patients (80 cases) after cystectomy were randomly divided into two groups to do prospective clinical observation. Patients in groups Leuprorelin and Tritorelin were iv injected with Leuprorelin 3.75 mg and Tritorelin 3.75 mg, respectively for 3—6 months and 28—30 d once. The side effects in the two groups were observed, and the follicle-stimulating hormone (FSH), luteotropic hormone (LH), and estradiol (E2) levels on days 0, 7, and 21 were determined. Results Patients in Triptorelin (33 cases) and Leuprorelin (27 cases) completed the therapy. Side effects such as hot flashes, night sweats, acne, muscle pain, joint pain, headache, abnormal vaginal bleeding happened in patients in the treatment groups. Acne, headache incidence showed the significant difference between the two groups (P＜0.01, P＜0.05). Sex hormone levels in patients of the two groups showed no significant difference (P＞0.05); FSH, LH, and E2 levels on the day 7 after administration all had the significance (P＜0.01, P＜0.05). FSH and E2 levels in the two treatment groups both had the significance (P＜0.01). Conclusion The side effects and hormone level in Triptorelin and Leuprorelin are different in the treatment of EMs. Leuprorelin treatment could slowly decrease the hormone level and the side effect is low. Individualized treatment should be carried out according to the characteristics of the different preparations in clinic.
