目的 分析天津市中心婦產(chǎn)科醫(yī)院抗菌藥物處方的使用情況，指導(dǎo)臨床合理用藥。方法 抽取2012—2013年天津市中心婦產(chǎn)科醫(yī)院門急診抗菌藥物處方，并分析抗菌藥物的使用及不合格處方情況。結(jié)果 2012年門急診抗菌藥物的使用率分別為8.28%、13.94%；2013年分別為7.98%、9.66%。2012、2013年被點(diǎn)評(píng)的抗菌藥物處方分別為5 791、4 783張，其中處方不合格率分別為28.6%、17.6%。不合格處方主要為臨床診斷指證不明確和選藥不恰當(dāng)，其中2012、2013年臨床診斷指證不明確處方分別為1 379、575張，在不合格處方中不明確率分別為83.2%、68.3%；選藥不恰當(dāng)處方分別為277、267張，在不合格處方中不當(dāng)率分別為16.8%、31.7%。結(jié)論 天津市中心婦產(chǎn)科醫(yī)院的抗菌藥物使用基本合理，但仍存在需要改正的問題，以后還需繼續(xù)加大力度宣傳抗菌藥物的合理應(yīng)用，保證患者的用藥安全。

Objective To analyze the usage of antibacterial prescriptions in Tianjin Central Hospital of Gynecology Obstetrics, in order to guide the clinical rational drug use. Methods The antibacterial prescriptions in Tianjin Central Hospital of Gynecology Obstetrics from 2012 to 2013 were selected, and the usage of antibacterials and unqualified prescriptions were analyzed.Results The usage rates of antibacterials in outpatient and emergency department were 8.28% and 13.94% in 2012; 7.98% and 9.66% in 2013, respectively. The numbers of antibacterial prescriptions reviewed in 2012 and 2013 were 5 791 and 4 783, and the ratios of unqualified prescriptions were 28.6% and 17.6%, respectively. The unqualified prescriptions were mainly that the evidence of clinical diagnosis was not clear and the choice of antibacterials was inappropriate. The numbers of antibacterials in 2012 and 2013 whose evidence of clinical diagnosis was not clear were 1 379 and 575, and the ratios were 83.2% and 68.3%, respectively, in the unqualified prescriptions. The numbers of antibacterials whose choice of antibacterials was inappropriate were 277 and 267, and the ratios were 16.8% and 31.7%, respectively, in the unqualified prescriptions. Conclusion The utilization of antibacterials in Tianjin Central Hospital of Gynecology Obstetrics is basically rational. However, there are still problems and shortcomings. So it needs to promote the reasonable application of antibacterials to further ensure the safety of medication in patients.
