目的 觀察孟魯司特聯(lián)合免疫球蛋白治療兒童腹型過(guò)敏性紫癜的療效。方法 選取2013年6月—2014年12月在中山市中醫(yī)院兒科接受治療的腹型過(guò)敏性紫癜患者80例,隨機(jī)分為對(duì)照組和治療組,每組各40例。對(duì)照組靜脈滴注凍干靜注人免疫球蛋白,0.4 g/(kg·d)。治療組在對(duì)照組治療基礎(chǔ)上口服孟魯司特鈉咀嚼片,2～5歲4 mg/d、6～11歲5 mg/d。兩組均連續(xù)治療10 d。觀察兩組的臨床療效,同時(shí)比較兩組嘔吐緩解時(shí)間、腹痛緩解時(shí)間、血便消失時(shí)間、皮疹開始消退時(shí)間、皮疹完全消退時(shí)間、住院時(shí)間。結(jié)果 治療后,對(duì)照組和治療組的總有效率分別為72.5%、92.5%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。兩組嘔吐緩解時(shí)間、腹痛緩解時(shí)間相比差異無(wú)統(tǒng)計(jì)學(xué)意義。治療組血便消失時(shí)間、皮疹開始消退時(shí)間、皮疹完全消退時(shí)間、住院時(shí)間均明顯短于對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論 孟魯司特聯(lián)合人免疫球蛋白治療兒童腹型過(guò)敏性紫癜具有較好的臨床療效,可顯著縮短各種臨床癥狀消失時(shí)間,具有一定的臨床推廣應(yīng)用價(jià)值

Objective To explore the clinical effect of montelukast combined with immunoglobulin in treatment of abdominal type allergic purpura in children. Methods Patients (80 cases) with abdominal type allergic purpura in Department of Pediatrics of Hospital of Traditional Chinese Medicine of Zhongshan from June 2013 to December 2014 were randomly divided into control and treatment groups, and each group had 40 cases. The patients in the control group were iv administered with Human Immunoglobulin for intravenous injection, 0.4 g/kg·d. The patients in the treatment group were po administered with Montelukast Sodium Chewable Tablets on the basis of control group, 2 — 5 years old with 4 mg/d, 6 — 11 years old with 5 mg/d. The patients in two groups were treated for 10 d. After treatment, the clinical efficacy was evaluated, relief times of vomiting and abdominal pain, bloody stools disappeared time, rash began to fade time, rash dissipated time, and time of hospital stay in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 72.5% and 92.5%, respectively, and there were differences between two groups (P < 0.05). After treatment, there was no statistically significant difference about relief times of vomiting and abdominal pain in two groups. Bloody stools disappeared time, rash began to fade time, rash dissipated time, and time of hospital stay in treatment group were shorter than those in the control group, and there were differences between two groups (P < 0.05). Conclusion Montelukast combined with immunoglobulin has good clinical effect in treatment of abdominal type allergic purpura in children, and can significantly shorten the various clinical symptoms disappear time, which has a certain clinical application value.
