目的 觀察莉芙敏片聯(lián)合戊酸雌二醇片和黃體酮膠囊治療圍絕經(jīng)期綜合征的臨床療效。方法 選取2013年6月-2015年6月屯昌縣人民醫(yī)院婦科收治的圍絕經(jīng)期綜合征患者88例,按不同治療方案分為治療組和對(duì)照組,每組各44例。對(duì)照組口服戊酸雌二醇片,1 mg/次,1次/d,治療一個(gè)月后開始服用黃體酮膠囊,2粒/次,1次/d。治療組在對(duì)照組用藥基礎(chǔ)上給予莉芙敏片,1片/次,2次/d。兩組患者均治療3個(gè)月。觀察兩組的臨床療效,同時(shí)比較兩組患者治療前后的Kupperman指數(shù)(KMI)評(píng)分、陰道萎縮癥狀評(píng)分、促卵泡生成素(FSH)及雌二醇(E₂)水平。結(jié)果 治療后,對(duì)照組和治療組的總有效率分別為86.36%、93.73%,兩組總有效率比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。治療后,兩組患者潮熱汗出、感覺異常、失眠、抑郁、皮膚瘙癢、骨關(guān)節(jié)痛評(píng)分及KMI總評(píng)分均顯著降低,同組治療前后差異具有統(tǒng)計(jì)學(xué)意義(P<0.05);且治療組這些觀察指標(biāo)明顯優(yōu)于對(duì)照組,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05)。治療后,兩組患者陰道干澀、陰道瘙癢、性交痛評(píng)分及陰道萎縮癥狀總評(píng)分均顯著降低,同組治療前后差異具有統(tǒng)計(jì)學(xué)意義(P<0.05);且治療組這些觀察指標(biāo)明顯好于對(duì)照組,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05)。治療后,兩組患者FSH顯著降低,和E₂顯著升高,同組治療前后差異具有統(tǒng)計(jì)學(xué)意義(P<0.05);且治療組這些觀察指標(biāo)的改善程度優(yōu)于對(duì)照組,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論 莉芙敏片聯(lián)合戊酸雌二醇片和黃體酮膠囊治療圍絕經(jīng)期綜合征的療效顯著,可以改善患者血清FSH和E2水平,不良反應(yīng)少,具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To observe the clinical effect of Remifemin Tablets combined with Estradiol Valerate Tablets and Progesterone Capsules in treatment of perimenopausal syndrome.Methods Patients (88 cases) with perimenopausal syndrome in Tunchang People's Hospital from June 2013 to June 2015 were enrolled in this study. According to the different treatment plan, patients were divided into control and treatment groups, and each group had 44 cases. The patients in the control group were poadministered with Estradiol Valerate Tablets, 1 mg/time, once daily. After treatment for one month, the patients were poadministered with Progesterone Capsules, 2 grains/times, once daily. The patients in the treatment group were poadministered with Remifemin Tablets on the basis of control group, 1 tablet/time, twice daily. The patients in two groups were treated for three months. After treatment, clinical efficacy was evaluated, and KMI scores, vaginal atrophy symptom scores, and levels of FSH and E₂ in two groups were compared before and after treatment.Results After treatment, the efficacies in the control and treatment groups were 86.36% and 93.73%, respectively, and there was significant difference between groups (P<0.05). After treatment, the scores of hot flushes and sweating, paresthesia, insomnia, depression, itchy skin, and joint pain, and total KMI score in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). After treatment, the scores of colpoxerosis, itchy vaginal, intercourse pain, and vaginal atrophy symptom in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). After treatment, the FSH in two groups was significantly decreased, but the E₂ in two groups were significantly increased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05).Conclusion Remifemin Tablets combined with Estradiol Valerate Tablets and Progesterone Capsules has clinical curative effect in the treatment of perimenopausal syndrome, and can improve the levels of FSH and E₂ in patients, and less adverse reactions, which has a certain clinical application value.
