目的 探討口炎清顆粒聯(lián)合曲安奈德治療口腔扁平苔蘚的臨床療效。方法 回顧性分析2014年1月-2015年10月在中國(guó)醫(yī)科大學(xué)附屬口腔醫(yī)院接受治療的口腔扁平苔蘚患者臨床資料78份,隨機(jī)分為對(duì)照組(39例)和治療組(39例)。所有患者均進(jìn)行基礎(chǔ)治療以減輕對(duì)口腔黏膜的刺激。對(duì)照組病灶黏膜基底部注射醋酸曲安奈德注射液,10 mg溶于2%利多卡因2.5 mL中,1次/周。治療組在對(duì)照組基礎(chǔ)上口服口炎清顆粒,2袋/次,2次/d。兩組患者均連續(xù)治療4周。比較兩組的臨床療效,同時(shí)比較兩組體征評(píng)分、疼痛評(píng)分和生活質(zhì)量評(píng)分變化,觀察兩組治療前后血清IL-2、IL-4和TNF-α水平變化。結(jié)果 治療后,對(duì)照組和治療組的總有效率為84.62%、97.44%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。治療后,兩組體征評(píng)分、疼痛評(píng)分和生活質(zhì)量評(píng)分均較同組治療前降低(P<0.05),且治療組比對(duì)照組降低的更顯著(P<0.05)。治療后兩組血清IL-2、IL-4和TNF-α水平均較同組治療前降低(P<0.05),且治療組降低的更顯著(P<0.05)。結(jié)論 口炎清顆粒聯(lián)合曲安奈德治療口腔扁平苔蘚療效顯著,可明顯降低患者血清炎性因子水平,有效改善患者臨床癥狀,具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To explore the clinical effect of Kouyanqing Granules combined with triamcinolone acetonide in treatment of oral lichen planus. Methods The clinical data of 78 patients with oral lichen planus in China Medical University Stomatology Hospital from January 2014 to October 2015 were retrospectively analyzed, and were randomly divided into control group (39 cases) and treatment group (39 cases). All patients were given basic treatment to reduce the stimulation to oral mucosa. The patients in the control group were injected with Triamcinolone Acetonide Acetate Injection in lesion mucosal basement, 10 mg dissolved in 2% lidocaine 2.5 mL, once a week. The patients in the treatment group were po administered with Kouyanqing Granules on the basis of the control group, 2 bags/time, twice daily. The patients in two groups were treated for 4 weeks. After treatment, clinical efficacies in two groups were evaluated. And the changes of scores of physical signs, pain and life quality were compared. In the same time, the levels of serum IL-2, IL-4, and TNF-α in two groups before and after treatment were determined. Results After treatment, the total efficacies in the control group and treatment group were 84.62% and 97.44%, respectively, and there were differences between two groups (P < 0.05). After treatment, the scores of physical signs, pain and life quality in two groups were significantly decreased (P < 0.05), and the scores in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P < 0.05). After treatment, the levels of serum IL-2, IL-4, and TNF-α in two groups weresignificantly decreased (P < 0.05), and the reductions in treatment group were more obvious, with significant difference between two groups (P < 0.05). Conclusion Kouyanqing Granules combined with triamcinolone acetonide has a significant curative effect in treatment of oral lichen planus, can significantly decrease the levels of serum inflammatory cytokines, and effectively improve clinical symptoms, which has a certain clinical application value.
