目的 觀察血必凈注射液聯(lián)合去甲腎上腺素治療感染性休克的臨床療效。方法 選取2013年8月-2015年3月東方市人民醫(yī)院重癥醫(yī)學(xué)科收治的感染性休克患者66例，隨機(jī)分為對照組和治療組，每組各33例。對照組靜脈滴注重酒石酸去甲腎上腺素注射液0.1 μg/(kg·min)，1次/d；治療組在對照組基礎(chǔ)上靜脈滴注血必凈注射液，100 mL/次，2次/d。兩組均連續(xù)治療14 d。觀察兩組的臨床療效，比較兩組患者的血流動力學(xué)及炎性因子的變化。結(jié)果 治療后，對照組和治療組的總有效率分別為81.82%、96.97%，兩組比較差異有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組患者的心臟指數(shù)（CI）、平均動脈壓（MAP）、中心靜脈壓（CVP）、血氧飽和度（SpO₂）及靜脈血氧飽和度（SvO₂）明顯升高，同組治療前后差異有統(tǒng)計學(xué)意義（P<0.05）；且治療組這些觀察指標(biāo)的升高程度明顯優(yōu)于對照組，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組患者的乳酸（LA）、C反應(yīng)蛋白（CPR）及血清降鈣素原（PCT）的水平均明顯降低，同組治療前后差異有統(tǒng)計學(xué)意義（P<0.05）；且治療組這些觀察指標(biāo)的降低程度明顯優(yōu)于對照組，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。結(jié)論 血必凈注射液聯(lián)合去甲腎上腺素治療感染性休克具有較好的臨床療效，能抑制炎癥反應(yīng)，改善血流動力學(xué)，具有一定的臨床推廣應(yīng)用價值。

Objective To observe the effect of Xuebijing Injection combined with norepinephrine in treatment of septic shock. Methods Patients (66 cases) with septic shock in Department of Intensive Care Unit of Dongfang People's Hospital from August 2013 to May 2015 were randomly divided into the control and treatment groups, and each group had 33 cases. Patients in the control group were iv administered with Noradrenaline Bitartrate Injection, infusion speed 0.1 μg/(kg·min), once daily. Patients in the treatment group were iv administered with Xuebijing Injection on the basis of control group, 100 mL/time, twice daily. Patients in two groups were treated for 14 d. After treatment, the clinical efficacies were evaluated, and the changes of hemodynamic indexes and inflammatory factors in two groups were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 81.82% and 96.97%, respectively, and there was difference between two groups (P<0.05). After treatment, CI, MAP, CVP, SpO₂, and SvO₂ in two groups were significantly increased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly higher than those in the control group, with significant difference between two groups (P<0.05). After treatment, the levels of LA, CPR, and PCT in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). Conclusion Xuebijing Injection combined with norepinephrine has the clinical curative effect in treatment of septic shock, and inhibit inflammatory reaction, improve hemodynamics, which has a certain clinical application value.
