目的 研究大珠紅景天注射液聯(lián)合辛伐他汀治療慢性阻塞性肺疾病合并肺動脈高壓的臨床療效。方法 選擇2014年1月-2015年6月在保定市第二醫(yī)院就診的慢性阻塞性肺疾病合并肺動脈高壓患者100例，隨機(jī)分為對照組和治療組，每組各50例。對照組口服辛伐他汀片，20 mg/次，1次/d；治療組在對照組治療基礎(chǔ)上靜脈滴注大株紅景天注射液，10 mL/次加入5%葡萄糖注射液250 mL，1次/d。兩組均連續(xù)治療14 d。觀察兩組的臨床療效，同時比較兩組治療前后用力肺活量（FVC）、第1秒用力呼氣容積（FEV₁）、肺活量百分比（FEV₁/FVC）、氧分壓（pO₂）、二氧化碳分壓（pCO₂）、血氧飽和度（SaO₂）、C-反應(yīng)蛋白（CRP）、B型鈉肽（NT-proBNP）、血漿內(nèi)皮素-1（ET-1）、平均肺動脈壓（mPAP）和呼吸困難指數(shù)評分的變化情況。結(jié)果 治療后，對照組和治療組總有效率分別為62.0%、82.0%，兩組比較差異有統(tǒng)計學(xué)意義（P<0.05）。治療后，兩組FEVI、FEV¹/FVC、pO₂、SaO₂均顯著升高，pCO₂、NT-proBNP、CRP、ET-1、mPAP、呼吸困難評分顯著降低，同組治療前后差異有統(tǒng)計學(xué)意義（P<0.05）。治療后，治療組這些觀察指標(biāo)的改善程度明顯優(yōu)于對照組，兩組比較差異具有統(tǒng)計學(xué)意義（P<0.05）。結(jié)論 大株紅景天注射液聯(lián)合辛伐他汀治療慢性阻塞性肺疾病合并肺動脈高壓具有較好的臨床療效，可顯著改善患者的肺功能、肺動脈高壓癥狀以及機(jī)體炎癥反應(yīng)，具有一定的臨床推廣應(yīng)用價值。

Objective To study the effect of Sofren Injection combined with simvastatin in treatment of chronic obstructive pulmonary disease with pulmonary hypertension. Methods Patients (100 cases) with chronic obstructive pulmonary disease with pulmonary hypertension in the Second Hospital of Baoding from January 2014 to June 2015 were randomly divided into control and treatment groups, and each group had 50 cases. The patients in the control group were po administered with Simvastatin Tablets, 20 mg/time, once daily. The patients in the treatment group were iv administered with Sofren Injection on the basis of the control group, 10 mL/time added into 5% glucose injection 250 mL, once daily. Patients in two groups were treated for 14 d. After treatment, the clinical efficacies were evaluated, and the changes of FVC, FEV₁, FEV₁/FVC, pO₂, pCO₂, SaO₂, CRP, NT-proBNP, ET-1, mPAP, and dyspnea index scores in two groups before and after treatment were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 62.0% and 82.0%, respectively, and there were differences between two groups (P<0.05). After treatment, FEVI, FEV¹/FVC, pO₂, and SaO₂ in two groups were significantly increased, and pCO₂, NT-proBNP, CRP ET-1, mPAP, and dyspnea index scores were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the observational indexes in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). Results Sofren Injection combined with simvastatin has clinical curative effect in treatment of chronic obstructive pulmonary disease with pulmonary hypertension, and can significantly improve lung function, pulmonary hypertension symptoms, and inflammatory response, which has a certain clinical application value.
