目的 探討參芪健胃顆粒聯(lián)合替普瑞酮治療慢性萎縮性胃炎的臨床療效。方法 選取2015年4月-2016年4月在子洲縣中醫(yī)院接受治療的慢性萎縮性胃炎患者82例，按照治療方法的差別分成對(duì)照組（41例）和治療組（41例）。對(duì)照組口服替普瑞酮膠囊，1粒/次，3次/d。治療組在對(duì)照組的基礎(chǔ)上口服參芪健胃顆粒，1袋/次，3次/d。兩組患者均連續(xù)治療4周。比較兩組臨床療效、中醫(yī)證候積分、白介素-6（IL-6）、IL-8、腫瘤壞死因子-α（TNF-α）、超氧化酶歧化酶（SOD）、NO、丙二醛（MDA）、內(nèi)皮素（ET）、表皮生長(zhǎng)因子（EGF）和降鈣素基因相關(guān)肽（CGRP）水平。結(jié)果 治療后，對(duì)照組和治療組總有效率分別為80.49%和95.12%，兩組比較差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組患者胃刺痛、倦怠乏力、胃脘脹滿和食少納呆癥候評(píng)分均較同組治療前顯著降低（P<0.05）；且治療后治療組這些癥候評(píng)分顯著低于對(duì)照組（P<0.05）。治療后，兩組IL-6、IL-8和TNF-α水平均低于同組治療前（P<0.05）；且治療后治療組這些觀察指標(biāo)顯著低于對(duì)照組（P<0.05）。治療后，兩組患者血清SOD和NO水平明顯升高，而MDA水平則降低，同組治療前后差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）；且治療組上述觀察指標(biāo)改善程度優(yōu)于對(duì)照組（P<0.05）。治療后，兩組患者血清胃腸激素ET和EGF水平明顯降低，而CGRP水平明顯升高，同組治療前后差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）；且治療組上述指標(biāo)優(yōu)于對(duì)照組（P<0.05）。結(jié)論 參芪健胃顆粒聯(lián)合替普瑞酮治療慢性萎縮性胃炎效果顯著，可明顯改善患者臨床癥狀、機(jī)體氧化應(yīng)激狀態(tài)和胃腸激素水平，降低血清炎性因子水平，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To explore the clinical effect of Shenqi Jianwei Granules combined with teprenone in treatment of chronic atrophic gastritis. Methods Patients (82 cases) with chronic atrophic gastritis in Zizhou County Hospital of TCM from April 2015 to April 2016 were divided into the control (41 cases) and treatment (41 cases) groups based on different treatments. The patients in the control group were po administered with Teprenone Capsules, 1 grain/time, three times daily. The patients in the treatment group were po administered with Shenqi Jianwei Granules on the basis of the control group, 1 bag/time, three times daily. The patients in two groups were treated for 4 weeks. After treatment, the clinical efficacies were evaluated, and the TCM syndromes scores and the levels of IL-6, IL-8, TNF-α, SOD, NO, MDA, ET, EGF, and CGRP in two groups before and after treatment were compared. Results After treatment, the clinical efficacies in the control and treatment groups were 80.49% and 95.12%, respectively, and there were differences between two groups (P<0.05). After treatment, the TCM syndromes scores of stomach pain, lassitude, gastric distention, and food less anorexia in two groups were significantly decreased (P<0.05). And these TCM syndromes scores in the treatment group were significantly lower than those in the control group (P<0.05). After treatment, IL-6, IL-8, and TNF-α levels in two groups were significantly decreased (P<0.05). And the observational indexes in the treatment group were significantly lower than those in the control group (P<0.05). After treatment, SOD and NO levels in two groups were significantly increased, and MDA was significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the improvement of these observational indexes in the treatment group was significantly better than those in the control group (P<0.05). After treatment, ET and EGF levels in two groups were significantly decreased, and CGRP levels were significantly increased, and the difference was statistically significant in the same group (P<0.05). And these observational indexes in the treatment group were better than those in the control group (P<0.05). Conclusion Shenqi Jianwei Granules combined with teprenone has a significant clinical effect in treatment of chronic atrophic gastritis, can significantly improve the clinical symptoms, the stress state and gastrointestinal hormone oxidation, and reduce serum levels of inflammatory factors, which has a certain clinical application value.
