[摘要]
目的 探討小牛血清去蛋白注射液聯(lián)合奧拉西坦治療腦挫裂傷的臨床療效。方法 選取深圳市寶安區(qū)中心醫(yī)院2015年8月-2016年8月收治的120例腦挫裂傷患者為研究對(duì)象,依據(jù)用藥差別分為對(duì)照組(60例)和治療組(60例)。對(duì)照組靜脈滴注奧拉西坦注射液,4.0 g加入到生理鹽水250 mL中,1次/d;治療組在對(duì)照組的基礎(chǔ)上靜脈滴注小牛血清去蛋白注射液,30 mL加入到5%的葡萄糖注射液250 mL中,1次/d。兩組患者均連續(xù)治療4周。評(píng)價(jià)兩組患者臨床療效,同時(shí)比較治療前后兩組簡易智力狀態(tài)檢查量表(MMSE)、格拉斯哥昏迷評(píng)分(GCS)和美國國立衛(wèi)生研究院卒中量表(NIHSS)評(píng)分以及血清細(xì)胞因子變化。結(jié)果 治療后,對(duì)照組和治療組的總有效率分別為81.67%、96.67%,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。兩組MMSE和GCS評(píng)分均升高,NIHSS評(píng)分降低,同組治療前后比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05);且治療后治療組的上述評(píng)分均優(yōu)于對(duì)照組,兩組比較差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。兩組血清白介素-6(IL-6)、腫瘤壞死因子-a(TNF-α)、粒細(xì)胞-巨噬細(xì)胞集落刺激因子(GM-CSF)、髓過氧化物酶(MPO)水平均顯著降低(P<0.05);且治療后治療組的上述血清細(xì)胞因子均低于對(duì)照組,兩組比較差異具有統(tǒng)計(jì)學(xué)意義(P<0.05)。結(jié)論 小牛血清去蛋白注射液聯(lián)合奧拉西坦注射液治療腦挫裂傷療效顯著,有利于降低血清相關(guān)細(xì)胞因子水平,促進(jìn)神經(jīng)功能恢復(fù),具有一定的臨床推廣應(yīng)用價(jià)值。

[Abstract]
Objective To investigate the clinical effects of Deproteinised Calf Blood Serum Injection combined with oxiracetam in treatment of cerebral contusion and laceration. Methods Patients (120 cases) with cerebral contusion and laceration in Shenzhen Baoan District Central Hospital from August 2015 to August 2016 were randomly divided into control (60 cases) and treatment (60 cases) groups. Patients in the control group were iv administered with Oxiracetam Injection, 4.0 g added into normal saline 250 mL, once daily. Patients in the treatment group were iv administered with Deproteinised Calf Blood Serum Injection, 30 mL added into 5% glucose solution 250 mL, once daily. Patients in two groups were treated for 4 weeks. After treatment, the clinical efficacy was evaluated, and the MMSE, GCS, NIHSS score, and serum cytokines in two groups before and after treatment were compared. Results After treatment, the clinical efficacy in the control and treatment groups were 81.67% and 96.67%, respectively, and there were differences between two groups (P<0.05). MMSE and GCS score in two groups were significantly increased, but NIHSS score were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the scores in the treatment group were significantly better than those in the control group, with significant difference between two groups (P<0.05). IL-6, TNF-α, GM-CSF, and MPO levels in two groups were significantly decreased, and the difference was statistically significant in the same group (P<0.05). And the serum cytokine levels in the treatment group were significantly lower than those in the control group, with significant difference between two groups (P<0.05). Conclusion Deproteinised Calf Blood Serum Injection combined with oxiracetam has clinical curative effect in treatment of cerebral contusion and laceration, and can decrease serum cytokine levels and improve neurological function, which has a certain clinical application value.
