目的 探討采用槐耳顆粒聯(lián)合替吉奧治療晚期結(jié)直腸癌患者的臨床效果。方法 選取天津醫(yī)科大學(xué)腫瘤醫(yī)院于2015年8月-2016年8月收治的晚期結(jié)直腸癌患者111例，隨機(jī)分成對(duì)照組（55例）和治療組（56例）。對(duì)照組患者口服替吉奧膠囊，患者體表面積<1.25 m²，40 mg/次，若體表面積1.25~1.50 m²，50 mg/次，若體表面積>1.5 m²，60 mg/次，早晚各1次，連續(xù)服用14 d，然后停藥7 d為1個(gè)療程。治療組患者在對(duì)照組的基礎(chǔ)上口服槐耳顆粒，1袋/次，3次/d，21 d為1個(gè)療程。兩組患者均連續(xù)治療3個(gè)療程。評(píng)價(jià)兩組患者臨床療效，同時(shí)比較治療前后兩組患者KPS和QOL評(píng)分以及血清標(biāo)志物和免疫指標(biāo)水平變化。結(jié)果 治療后，對(duì)照組患者客觀緩解率和疾病控制率分別為34.54%、65.45%，均分別低于治療組的57.14%、89.29%，兩組比較差異均具有統(tǒng)計(jì)學(xué)意義（P<0.05）。治療后，兩組患者KPS和QOL評(píng)分均顯著升高（P<0.05）；且治療組患者KPS和QOL評(píng)分明顯高于對(duì)照組患者（P<0.05）。治療后，兩組患者癌胚抗原（CEA）、IL-9以及轉(zhuǎn)化生長(zhǎng)因子-β（TGF-β）血清水平均顯著降低（P<0.05）；且治療組患者各血清標(biāo)志物水平均明顯低于對(duì)照組（P<0.05）。治療后，兩組患者IgA、IgM和IgG血清水平均顯著升高（P<0.05）；且治療組患者免疫指標(biāo)均明顯高于對(duì)照組（P<0.05）。結(jié)論 槐耳顆粒聯(lián)合替吉奧膠囊治療晚期結(jié)直腸癌療效顯著，能夠改善患者血清標(biāo)志物和免疫指標(biāo)水平，且安全性高，具有一定的臨床推廣應(yīng)用價(jià)值。

Objective To explore the clinical effect of Huaier Granules combined with tegafur, gimeracil and oteracil potassium in treatment of advanced colorectal cancer. Methods Patients (111 cases) with advanced colorectal cancer in Tianjin Medical University Cancer Hospital from August 2015 to August 2016 were divided into control (55 cases) and treatment (56 cases) groups. Patients in the control group were po administered with Tegafur Gimeracil Oteracil Potassium Capsules, 40 mg/time for the patients of body surface area less than 1.25 m², and 50 mg/time for the patients of body surface area less than 1.25-1.50 m², and 60 mg/time for the patients of body surface area more than 1.5 m², twice daily, and one course of treatment was 14 d of treatment and 7 d of withdrawal. Patients in the treatment group were po administered with Huaier Granules on the basis of the control group, 1 bag/time, three times daily, one course of treatment was 21 d. Patients in two groups were treated for 3 courses of treatment. After treatment, the clinical efficacy was evaluated, and the change of KPS and QOL scores, serum markers and immune indexes in two groups before and after treatment was compared. Results After treatment, objective remission rate and disease control rate in the control group were 34.54% and 65.45%, which were significantly lower than 57.14% and 89.29% in the treatment group, respectively, and there were differences between two groups (P<0.05). After treatment, the KPS and QOL scores in two groups were significantly increased (P<0.05). And the KPS and QOL scores in the treatment group were obviously higher than that in the control group (P<0.05). After treatment, the serum CEA, IL-9 and TGF-β levels in two groups were significantly decreased (P<0.05). And the serum markers levels in the treatment group were obviously lower than those in the control group (P<0.05). After treatment, the serum IgA, IgM and IgG levels in two groups were significantly increased (P<0.05). And the improvement of these immune indexes in the treatment group was obviously better than that in the control group (P<0.05). Conclusion Huaier Granules combined with tegafur, gimeracil and oteracil potassium has significant effect in treatment of advanced colorectal cancer significantly, can significantly improve serum markers and immune indicators with high security, which has a certain clinical application value.
