[摘要]
目的 探討桉檸蒎腸溶軟膠囊聯(lián)合噻托溴銨奧達特羅吸入噴霧劑對治療慢性阻塞性肺疾?。–OPD)穩(wěn)定期患者的肺功能及氧化應(yīng)激水平的影響。方法 選擇2019年2月-2021年2月在南陽市中心醫(yī)院治療的136例穩(wěn)定期COPD患者,根據(jù)用藥的差別分為對照組和治療組,每組各68例。對照組吸入噻托溴銨奧達特羅吸入噴霧劑,2噴/次,1次/d;治療組在對照組的基礎(chǔ)上口服桉檸蒎腸溶軟膠囊,0.3 g/次,2次/d。兩組均經(jīng)14 d治療。觀察兩組患者臨床療效,比較治療前后兩組患者APACHE II評分、CAT評分、SGRQ評分、mMRC評分,血清丙二醛(MDA)、8-羥基脫氧鳥苷(8-OHdG)、脂質(zhì)過氧化物(LPO)、蛋白C(PC)、總抗氧化能力(TAC)和超氧化物歧化酶(SOD)水平,及肺功能第1秒用力呼氣容積(FEV1)、第1秒用力呼氣量占用力肺活量比值(FEV1/FVC)和FEV1占預(yù)計值百分比(FEV1%)水平。結(jié)果 經(jīng)治療,治療組臨床有效率明顯高于對照組(98.53%vs 82.35%,P<0.05)。經(jīng)治療,兩組APACHE II評分、CAT評分、SGRQ評分、mMRC評分均明顯下降(P<0.05),且治療組降低更明顯(P<0.05)。經(jīng)治療,兩組血清MDA、8-OHdG、LPO、PC水平均明顯下降,而SOD、TAC明顯升高(P<0.05),且治療組改善更明顯(P<0.05)。經(jīng)治療,兩組FEV1、FEV1%和FEV1/FVC均顯著升高(P<0.05),且治療組升高更明顯(P<0.05)。結(jié)論 桉檸蒎腸溶軟膠囊聯(lián)合噻托溴銨奧達特羅吸入噴霧劑治療穩(wěn)定期COPD不僅可以促進患者肺功能改善,還有利于患者機體氧化應(yīng)激水平的改善,促進呼吸困難情況的改善,提高患者生活質(zhì)量。

Objective To investigate the effects of Eucalyptol, Limonene and Pinene Enteric Soft Capsules combined with Tiotropium Bromide and Olodaterol Hydrochloride Inhalation Spray on pulmonary function and oxidative stress in patients with chronic obstructive pulmonary disease (COPD) in stable stage. Methods Patients (136 cases) with stable stage of COPD in Nanyang Central Hospital from February 2019 to February 2021 were divided into control and treatment groups based on different treatments, and each group had 68 cases. Patients in the control group were administered with Tiotropium Bromide and Olodaterol Hydrochloride Inhalation Spray, 2 press/time, once daily. Patients in the treatment group were po administered with Eucalyptol on the basis of the control group, Limonene and Pinene Enteric Soft Capsules, 0.3 g/time, twice daily. Patients in two groups were treated for 14 d. After treatment, the clinical efficacy was evaluated, the scores of APACHE II, CAT, SGRQ and mMRC, the serum levels of MDA, 8-OHdG, LPO, PC, SOD and TAC, the pulmonary function of FEV1, FEV1% and FEV1/FVC levels in two groups before and after treatment were compared. Results After treatment, the clinical effective rate of the treatment group was significantly higher than that of the control group (98.53% vs 82.35%, P<0.05). After treatment, the scores of APACHE II, CAT, SGRQ and mMRC score were significantly decreased in two groups, especially in the treatment group (P<0.05). After treatment, the serum levels of MDA, 8-OHdG, LPO and PC were significantly decreased, while SOD and TAC were significantly increased in two groups (P<0.05), and the improvement of the treatment group was more obvious (P<0.05). After treatment, FEV1, FEV1% and FEV1/FVC were significantly increased in two groups (P<0.05), especially in the treatment group (P<0.05). Conclusion Eucalyptol, Limonene and Pinene Enteric Soft Capsules combined with Tiotropium Bromide and Olodaterol Hydrochloride Inhalation Spray in treatment of stable stage of COPD can not only improve the lung function of patients, but also improve the level of oxidative stress, and improve dyspnea and improve the quality of life of patients.
