目的 探討雙骨三子膠囊聯(lián)合骨化三醇治療女性絕經(jīng)后骨質(zhì)疏松性髖部骨折的臨床效果。方法 選取2018年3月—2021年3月許昌仁和骨傷醫(yī)院收治的128例絕經(jīng)后骨質(zhì)疏松性髖部骨折患者，隨機(jī)分成對照組和治療組，每組各64例。對照組口服骨化三醇軟膠囊，1粒/次，2次/d。治療組在對照組基礎(chǔ)上口服雙骨三子膠囊，3粒/次，3次/d。兩組均連續(xù)治療6個(gè)月。觀察兩組患者臨床療效，比較治療前后兩組患者骨折臨床愈合時(shí)間，疼痛視覺模擬量表（VAS）評分，36項(xiàng)健康調(diào)查簡表（SF-36）總分，腰椎正位和髖部的骨密度（BMD），尿鈣與尿肌酐比值（U-Ca/Cr）和血清I型前膠原N端前肽（P1NP）、I型膠原羧基末端肽β特殊序列（β-CTX）、白細(xì)胞介素（IL）-6水平。結(jié)果 治療后，治療組臨床有效率為93.75%，顯著高于對照組的78.12%（P<0.05）。與對照組相比，治療組骨折臨床愈合時(shí)間顯著縮短（P<0.05）。治療后，兩組疼痛VAS評分均顯著降低，而SF-36總分則均顯著升高（P<0.05），且治療組改善更顯著（P<0.05）。治療后，兩組腰椎正位和髖部的BMD較治療前均顯著增加（P<0.05），且治療組比對照組增加更顯著（P<0.05）。治療后，兩組U-Ca/Cr和血清β-CTX、IL-6水平較治療前均顯著下降，而血清PINP水平顯著上升（P<0.05），且治療組這些指標(biāo)比對照組改善更顯著（P<0.05）。結(jié)論 雙骨三子膠囊聯(lián)合骨化三醇治療女性絕經(jīng)后骨質(zhì)疏松性髖部骨折的總體療效滿意，能安全有效地緩解患者疼痛，促進(jìn)骨折愈合，增加BMD，改善生活質(zhì)量。

Objective To investigate the clinical effect of Shuanggu Sanzi Capsules combined with calcitriol in treatment of postmenopausal women with osteoporotic hip fractures. Methods Patients (128 cases) with postmenopausal osteoporotic hip fracture in Xuchang Renhe Bone Injury Hospital from March 2018 to March 2021 were randomly divided into control and treatment group, and each group had 64 cases. Patients in the control group were po administered with Calcitriol Soft Capsules, 1 grain/time, twice daily. Patients in the treatment group were po administered with Shuanggu Sanzi Capsules on the basis of the control group, 3 grains/time, three times daily. Patients in two groups were treated for 6 weeks. After treatment, the clinical evaluation was evaluated, the fracture healing time, pain VAS score, SF-36 total score, BMD of lumbar vertebrae and hip, U-Ca/Cr and serum PINP, β-CTX and IL-6 levels in two groups before and after treatment were compared. Results After treatment, the clinical effective rate of the treatment group was 93.75%, which was significantly higher than that of the control group (78.12%) (P < 0.05). Compared with the control group, the clinical healing time of fracture in the treatment group was significantly shorter (P < 0.05). After treatment, the VAS score in two groups were significantly decreased, while the total score of SF-36 were significantly increased (P < 0.05), and the improvement in the treatment group was more significant (P < 0.05). After treatment, the BMD of lumbar spine and hip in two groups were significantly increased compared with that before treatment (P < 0.05), and the change in the treatment group was more significant than that in the control group (P < 0.05). After treatment, the levels of U-Ca/Cr, serum β-CTX and IL-6 in two groups were significantly lower than those before treatment, while the level of serum PINP was significantly increased (P < 0.05), and these indicators in the treatment group was significantly better than those in the control group (P < 0.05). Conclusion The overall curative effect of Shuanggu Sanzi Capsules combined with calcitriol in treatment of female postmenopausal osteoporotic hip fracture is satisfactory, which can safely and effectively relieve pain, promote fracture healing, increase BMD and improve quality of life.
