[摘要]
目的 探討消渴康顆粒聯(lián)合聚乙二醇洛塞那肽注射液治療2型糖尿病的臨床療效。方法 選取2020年2月—2022年1月在天津醫(yī)科大學(xué)第二醫(yī)院就診的104例2型糖尿病患者,按照隨機(jī)數(shù)字表法將所有患者分為對照組和治療組,每組52例。對照組皮下注射聚乙二醇洛塞那肽注射液,0.1 mg/次,1次/周。治療組在對照組基礎(chǔ)上口服消渴康顆粒,1袋/次,3次/d。兩組在治療30 d后統(tǒng)計療效。觀察兩組的臨床療效,比較兩組血糖指標(biāo)、血糖波動指標(biāo)以及脂肪型脂肪酸結(jié)合蛋白(A-FABP)、趨化素(chemerin)、白細(xì)胞介素-17(IL-17)水平。結(jié)果 治療后,治療組的總有效率為96.15%,高于對照組的總有效率82.69%,組間比較差異顯著(P<0.05)。治療后,兩組患者的空腹血糖(FPG)、糖化血紅蛋白(HbA1c)、餐后2 h血糖(2 h BG)顯著降低(P<0.05),治療組FPG、HbA1c、2 h BG比對照組更低(P<0.05)。治療后,兩組的平均血糖波動幅度(MAGE)、日間血糖平均絕對差(MODD)、血糖水平標(biāo)準(zhǔn)差(SDBG)水平均明顯降低(P<0.05),治療組MAGE、MODD、SDBG水平明顯低于對照組,組間差異有顯著意義(P<0.05)。治療后,兩組的血清A-FABP、chemerin、IL-17水平均明顯低于治療前(P<0.05);治療組患者的血清A-FABP、chemerin、IL-17水平低于對照組,差異有統(tǒng)計學(xué)意義(P<0.05)。結(jié)論 消渴康顆粒聯(lián)合聚乙二醇洛塞那肽注射液治療2型糖尿病的療效確切,可提高降糖療效,降低血糖波動,調(diào)節(jié)A-FABP、chemerin、IL-17水平,值得臨床推廣運(yùn)用。

Objective To investigate the clinical efficacy of Xiaokekang Granules combined with Polyethylene Glycol Loxenatide Injection in treatment of type 2 diabetes mellitus. Methods Patients (104 cases) with type 2 diabetes mellitus in the Second Hospital of Tianjin Medical University from February 2020 to January 2022 were randomly divided into control and treatment groups, and each group had 52 cases. Patients in the control group were sc administered with Polyethylene Glycol Loxenatide Injection, 0.1 mg/time, once weekly. Patients in the treatment group were po administered with Xiaokekang Granules on the basis of the control group, 1 bag/time, three times daily. Patients in two groups were treated for 30 d. After treatment, the clinical efficacies were evaluated, and the blood glucose indexes, the blood glucose fluctuation indexes, and the levels of A-FABP, chemerin, and IL-17 in two groups were compared. Results After treatment, the total effective rate of the treatment group was 96.15%, which was higher than 82.69% of the control group, and there was significant difference between two groups (P < 0.05). After treatment, the FPG, HbA1c, and 2 h BG in two groups were significantly decreased (P < 0.05), and the FPG, HbA1c and 2 h BG in the treatment group were lower than those in the control group (P < 0.05). After treatment, the levels of MAGE, MODD, and SDBG in two groups were significantly decreased (P < 0.05). The levels of MAGE, MODD, and SDBG in the treatment group were significantly lower than those in the control group (P < 0.05). After treatment, the serum levels of A-FABP, chemerin, and IL-17 in two groups were significantly lower than those before treatment (P < 0.05). The serum levels of A-FABP, chemerin, and IL-17 in the treatment group were significantly lower than those in the control group (P < 0.05). Conclusion Xiaokekang Granules combined with Polyethylene Glycol Loxenatide Injection has clinical curative effect in treatment of type 2 diabetes mellitus, which can improve the hypoglycemic effect, reduce the fluctuation of blood glucose, and adjust the levels of A-FABP, chemerin, and IL-17, it is worthy of clinical application.
