[摘要]
目的 分析恒古骨傷愈合劑聯(lián)合注射用骨肽治療脛骨平臺(tái)骨折的臨床研究。方法 選取2019年6月—2022年6月鄭州市第七人民醫(yī)院治療的143例脛骨平臺(tái)骨折患者,按照隨機(jī)數(shù)字表法將患者分為對(duì)照組(72例)和治療組(71例)。對(duì)照組患者在手術(shù)后靜脈滴注注射用骨肽,100 mg注射用骨肽加入0.9%氯化鈉注射液250 mL中,1次/d。治療組在對(duì)照組的治療基礎(chǔ)上口服恒古骨傷愈合劑,25 mL/次,隔日用藥一次。兩組治療時(shí)間為1個(gè)月。觀察兩組的臨床療效,比較臨床癥狀改善時(shí)間、血清創(chuàng)傷指標(biāo)和骨代謝指標(biāo)。結(jié)果 治療后,治療組患者膝關(guān)節(jié)恢復(fù)優(yōu)良率是90.14%,顯著高于對(duì)照組的76.39%(P<0.05)。治療后,治療組患者疼痛消失時(shí)間、下床活動(dòng)時(shí)間、骨折愈合時(shí)間均顯著短于對(duì)照組(P<0.05)。治療后,兩組患者白細(xì)胞介素-10(IL-10)、白細(xì)胞介素-1(IL-1)和C反應(yīng)蛋白(CRP)均較治療前增加(P<0.05),且治療后治療組IL-10、IL-1和CRP明顯低于對(duì)照組(P<0.05)。治療后,兩組骨保護(hù)素(OPG)、骨形態(tài)發(fā)生蛋白2(BMP-2)水平均較治療前顯著升高,而Ⅰ型膠原羧基末端肽(CTX-Ⅰ)水平均顯著下降(P<0.05);治療后,治療組患者骨代謝指標(biāo)改善優(yōu)于對(duì)照組(P<0.05)。水平。結(jié)論 恒古骨傷愈合劑聯(lián)合注射用骨肽治療脛骨平臺(tái)骨折具有較好的臨床療效,有助于更快改善臨床癥狀,減輕炎癥反應(yīng),改善骨代謝水平,促進(jìn)膝關(guān)節(jié)功能恢復(fù)。

[Abstract]
Objective To investigate the therapeutic effect of Henggu Gushang Yuheji combined with Ossotide for injection in treatment of tibial plateau fracture. Methods A total of 143 patients with tibial plateau fracture treated in Zhengzhou Seventh People’s Hospital from June 2019 to June 2022 were selected and divided into control group (72 cases) and treatment group (71 cases) according to random number table method. Patients in the control group were iv administered with Ossotide for injection after surgery, 100 mg Ossotide for injection was added to 0.9% sodium chloride injection 250 mL, once daily. Patients in the treatment group were po administered with Henggu Gushang Yuheji, 25 mL/time, once every other day. The treatment time of the two groups was 1 month. The clinical efficacy of the two groups was observed, and the improvement time of clinical symptoms, serum trauma indexes and bone metabolism indexes were compared. Results After treatment, the recovery rate of knee joint in the treatment group was 90.14%, which was significantly higher than that in the control group (76.39%, P < 0.05). After treatment, the time of pain disappearance, the time of getting out of bed, and the time of fracture healing in the treatment group were significantly shorter than those in the control group (P < 0.05). After treatment, interleukin-10 (IL-10), interleukin-1 (IL-1) and C-reactive protein (CRP) were increased in both groups compared with before treatment (P < 0.05), and after treatment, IL-10, IL-1 and CRP in the treatment group were significantly lower than those in the control group (P < 0.05). After treatment, the levels of osteoprotectin (OPG) and bone morphogenetic protein 2 (BMP-2) were significantly increased in both groups compared with before treatment, but the levels of carboxyl terminal peptide of type Ⅰ collagen (CTX-Ⅰ) were significantly decreased in both groups (P < 0.05). After treatment, the improvement of bone metabolism indexes in treatment group was better than that in control group (P < 0.05). Conclusion Henggu Gushang Yuheji combined with Ossotide for injection has good clinical effect in treatment of tibial plateau fracture, and is helpful to improve clinical symptoms faster, reduce inflammation, improve bone metabolism and promote the recovery of knee function.
