目的 探討熱淋清顆粒聯(lián)合注射用頭孢哌酮鈉舒巴坦鈉治療急性腎盂腎炎的臨床療效。方法 選取2020年5月—2023年4月在唐山市中醫(yī)醫(yī)院就診的70例急性腎盂腎炎患者，根據(jù)計(jì)算機(jī)隨機(jī)排列將所有患者分為對(duì)照組和治療組，每組各35例。對(duì)照組靜脈滴注注射用頭孢哌酮鈉舒巴坦鈉，將3 g加入100 mL生理鹽水充分稀釋后，滴注30～60 min，1次/d。治療組在對(duì)照組基礎(chǔ)上口服熱淋清顆粒，2袋/次，3次/d。兩組患者連續(xù)治療2周。觀察兩組的總有效率，比較兩組患者的發(fā)熱、尿路刺激征、腰痛消失時(shí)間，急性生理學(xué)與健康狀況評(píng)分量表（APACHE Ⅱ）評(píng)分變化，以及超敏C反應(yīng)蛋白（hs-CRP）、白細(xì)胞介素-8（IL-8）、降鈣素原（PCT）、β₂-微球蛋白（β₂-MG）、胱抑素C（Cys-C）的水平。結(jié)果 治療后，治療組的總有效率94.29%明顯高于對(duì)照組的總有效率77.14%，組間比較差異顯著（P＜0.05）。治療后，治療組患者發(fā)熱、尿路刺激征、腰痛消失時(shí)間短于對(duì)照組，差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）。治療后，兩組的APACHE II評(píng)分低于治療前（P＜0.05），且治療組的APACHE II評(píng)分低于對(duì)照組（P＜0.05）。治療后，兩組的血清hs-CRP、IL-8、PCT水平低于治療前（P＜0.05），且治療組的血清hs-CRP、IL-8、PCT水平低于對(duì)照組，差異有統(tǒng)計(jì)學(xué)意義（P＜0.05）。治療后，兩組的β₂-MG、Cys-C水平低于治療前（P＜0.05），且治療組的β₂-MG、Cys-C水平低于對(duì)照組（P＜0.05）。結(jié)論 熱淋清顆粒聯(lián)合注射用頭孢哌酮鈉舒巴坦鈉可提高急性腎盂腎炎的療效，減輕病情嚴(yán)重程度和臨床癥狀，降低炎癥反應(yīng)，改善患者腎功能。

Objective To investigate the clinical effect of Relinqing Granules combined with Cefoperazone Sodium and Sulbactam Sodium for injection in treatment of acute pyelonephritis. Methods Patients (70 cases) with acute pyelonephritis in Tangshan Hospital of Traditional Chinese Medicine from May 2020 to April 2023 were divided into control and treatment groups according to the computer random arrangement method, and each group had 35 cases. Patients in the control group were iv administered with Cefoperazone Sodium and Sulbactam Sodium for injection, 3 g added into normal saline 100 mL, infusion 30 — 60 min, once daily. Patients in the treatment group were po administered with Relinqing Granules on the basis of the control group, 2 bags/time, three times daily. Patients in two groups were treated for 2 weeks. After treatment, the clinical efficacies were evaluated, and the disappearance time of fever, urinary tract irritation sign, and low back pain, APACHE II scores, the levels of hs-CRP, IL-8, PCT, β₂-MG, and Cys-C in two groups were compared. Results After treatment, the total effective rate of 94.29% in the treatment group was significantly higher than 77.14% in the control group, with significant differences between the groups (P < 0.05). After treatment, the disappearance time of fever, urinary tract irritation and low back pain in the treatment group was shorter than that in the control group, and the difference was statistically significant (P < 0.05). After treatment, APACHE II scores in two groups were lower than before treatment (P < 0.05), and APACHE II scores in the treatment group were lower than those in the control group (P < 0.05). After treatment, the serum levels of hs-CRP, IL-8, and PCT in two groups were lower than before treatment (P < 0.05), and the serum levels of hs-CRP, IL-8, and PCT in the treatment group were lower than those in the control group after treatment with statistical significance (P < 0.05). After treatment, the levels of β₂-MG and Cys-C in two groups were lower than before treatment (P < 0.05), and the levels of β₂-MG and Cys-C in the treatment group were lower than those in the control group (P < 0.05). Conclusion Relinqing Granules combined with Cefoperazone Sodium and Sulbactam Sodium for injection can improve the curative effect of acute pyelonephritis, reduce the severity of the disease and clinical symptoms, reduce inflammatory response, and improve patients’ kidney function.
