[摘要]
目的 探討痰熱清注射液聯(lián)合注射用替加環(huán)素治療醫(yī)院獲得性肺炎的臨床療效。方法 選取2022年3月—2023年11月在天津中醫(yī)藥大學第二附屬醫(yī)院就診的85例醫(yī)院獲得性肺炎患者,按隨機數(shù)字表法將患者分為對照組(42例)和治療組(43例)。對照組靜脈滴注注射用替加環(huán)素,首次靜滴劑量100 mg,1次/d,之后每12小時1次,每次50 mg,每次持續(xù)30~60 min。治療組在對照組基礎上靜滴痰熱清注射液,20 mL/次,1次/d。兩組持續(xù)治療10 d。比較兩組臨床療效、癥狀消失時間、病情程度和血清指標。結果 治療后,治療組的總有效率為95.35%,對照組的總有效率為80.95%,組間比較差異有統(tǒng)計學意義(P<0.05)。治療后,治療組高熱、咳嗽、肺啰音、肺部陰影消失時間均短于對照組(P<0.05)。治療后,兩組臨床肺部感染評分(CPIS)均明顯下降(P<0.05),治療組CPIS評分明顯低于對照組(P<0.05)。治療后,兩組患者的血清髓樣細胞觸發(fā)受體-1(TREM-1)、載脂蛋白E(ApoE)、降鈣素原(PCT)水平均低于治療前(P<0.05);治療組的血清TREM-1、ApoE、PCT水平低于對照組(P<0.05)。結論 痰熱清注射液聯(lián)合注射用替加環(huán)素可提高醫(yī)院獲得性肺炎的臨床療效,有效控制病情,改善臨床癥狀,降低炎癥反應。

[Abstract]
Objective To investigate the curative effect of Tanreqing Injection combined with Tigacycline for injection in treatment of hospital-acquired pneumonia. Methods A total of 85 patients with hospital-acquired pneumonia treated in the Second Affiliated Hospital of Tianjin University of Chinese Medicine from March 2022 to November 2023 were selected and divided into control group (42 cases) and treatment group (43 cases) according to random number table method. Patients in the control group were given intravenous Tigecycline for injection, the first dose of 100 mg, once daily, and then once every 12 h, 50 mg/time, for 30 to 60 min. Patients in the treatment group were given Tanreqing Injection on the basis of control group, 20 mL/time, once daily. Patients of two groups were treated for 10 d. The clinical efficacy, symptom disappearance time, disease degree, and serum indexes were compared between two groups. Results The total effective rate of patients in the treatment group was 95.35%, and that in the control group was 80.95%, and the difference between groups was significant (P < 0.05). The disappearance time of high fever, cough, lung rales, and lung shadows in the treatment group was shorter than that in the control group (P < 0.05). CPIS score in both groups were decreased significantly, and CPIS score of the treatment group was significantly lower than that in the control group (P < 0.05). After treatment, the serum levels of TREM-1, ApoE, and PCT in both groups were lower than that before treatment (P < 0.05), and the serum levels of TREM-1, ApoE, and PCT in the treatment group were lower than those in the control group (P < 0.05). Conclusion Tanreqing Injection combined with Tigacycline for injection can improve the curative effect of hospital-acquired pneumonia, effectively control the disease, improve the efficiency of symptom improvement, and reduce the inflammatory reaction.
