[摘要]
目的 探討西吡氯銨含片聯(lián)合米諾環(huán)素軟膏治療種植體周圍炎的臨床療效。方法 選取2022年10月—2024年11月唐山職業(yè)技術(shù)學(xué)院附屬醫(yī)院收治的98例患者,依據(jù)隨機(jī)數(shù)字法分為對(duì)照組和治療組,每組各49例。對(duì)照組患者牙周袋內(nèi)注滿米諾環(huán)素軟膏,禁食1 h,1次/周。在對(duì)照組的基礎(chǔ)上,治療組口服西吡氯銨含片,1片/次,4次/d。兩組患者均連續(xù)治療14 d。觀察兩組患者臨床療效,比較治療前后兩組患者菌斑指數(shù)評(píng)分(PLI)、齦溝出血指數(shù)評(píng)分(SBI)、牙齦指數(shù)評(píng)分(GI)評(píng)分,齦溝液中白細(xì)胞介素-1β(IL-1β)、黏蛋白-4(MUC-4)、腫瘤壞死因子-α(TNF-α)和基質(zhì)金屬蛋白酶-7(MMP-7)水平。結(jié)果 治療后,治療組總有效率(97.96%)明顯高于對(duì)照組的總有效率(81.63%,P<0.05)。治療后,兩組患者PLI、GI、SBI評(píng)分比治療前明顯下降(P<0.05),且治療后,與對(duì)照組對(duì)比,治療組PLI、GI、SBI評(píng)分下降更明顯(P<0.05)。治療后,兩組齦溝液中IL-1β、TNF-α、MMP-7水平比治療前明顯降低,而且MUC-4水平明顯升高(P<0.05),且治療后,治療組IL-1β、TNF-α、MMP-7和MUC-4水平明顯好于對(duì)照組(P<0.05)。結(jié)論 西吡氯銨含片與米諾環(huán)素軟膏協(xié)同治療種植體周圍炎療效顯著,能減輕機(jī)體炎癥損傷,有效改善牙周健康狀況。

[Abstract]
Objective To explore the clinical effect of cetylpyridinium chloride combined with minocycline in treatment of peri-implantitis. Methods Patients (98 cases) with peri-implantitis in Affiliated Hospital of Tangshan Vocational and Technical College from October 2022 to November 2024 were divided into control and treatment group according to the random number method, and each group had 49 cases. Patients in the control group were administered with Minocycline Hydrochloride Ointment in periodontal pockets, fasting for 1 h, once weekly. Patients in the treatment group were po administered with Cetylpyridinium Chloride Buccal Tablets on the basis of the control group, 1 tablet/time, four times daily. Patients in two groups were treated for 14 d. After treatment, the clinical evaluations were evaluated, and the scores of PLI, GI and SBI, the levels of IL-1β, TNF-α, MMP-7 and MUC-4 of gingival crevicular fluid in two groups before and after treatment were compared. Results After treatment, the effective rate of the treatment group (97.96%) was significantly higher than that of the control group (81.63%, P < 0.05). After treatment, the PLI, GI, and SBI scores in two groups were significantly decreased compared to before treatment (P < 0.05), and compared with the control group, the PLI, GI, and SBI scores of the treatment group were decreased more significantly after treatment (P < 0.05). After treatment, the levels of IL-1β, TNF-α, and MMP-7 in the gingival crevicular fluid in two groups were significantly reduced compared to before treatment, and the level of MUC-4 was significantly increased (P < 0.05). After treatment, the levels of IL-1β, TNF-α, MMP-7, and MUC-4 in the treatment group were significantly better than those in the control group (P < 0.05). Conclusion The synergistic treatment of Cetylpyridinium Chloride Buccal Tablets and Minocycline Hydrochloride Ointment has significant therapeutic effects, which can reduce inflammatory damage and effectively improve periodontal health.
