目的 探討槐杞黃顆粒聯(lián)合西咪替丁治療兒童IgA血管炎的臨床療效。方法 選取2020年1月1日—2022年12月31日成都兒童?？漆t(yī)院收治的120例IgA血管炎患兒，按照計(jì)算機(jī)隨機(jī)排列法將所有患兒分為對(duì)照組（60例）與治療組（60例）。對(duì)照組靜滴西咪替丁注射液，每次5 mg/kg，2次/d。治療組在對(duì)照組的基礎(chǔ)上溫水沖服槐杞黃顆粒，1袋/1次，2次/d。兩組患兒分別連續(xù)治療2周。觀察兩組患兒的臨床療效和癥狀消失時(shí)間，比較兩組治療前后腎功能、生化指標(biāo)、血清免疫指標(biāo)的變化。結(jié)果 治療組的總有效率（93.33%）高于對(duì)照組（80.00%，P＜0.05）。治療后，治療組皮膚紫癜消失時(shí)間、關(guān)節(jié)痛消失時(shí)間、腹痛消失時(shí)間顯著短于對(duì)照組（P＜0.05）。治療后，兩組血肌酐（Scr）、24 h尿蛋白量、尿沉渣紅細(xì)胞計(jì)數(shù)低于治療前（P＜0.05）；治療組患兒Scr、24 h尿蛋白量、尿沉渣紅細(xì)胞計(jì)數(shù)低于對(duì)照組（P＜0.05）。治療后，兩組IgA水平均比治療前低，IgM、IgG比治療前高（P＜0.05）；治療組患兒的IgA水平比對(duì)照組低，IgM、IgG比對(duì)照組高（P＜0.05）。結(jié)論 槐杞黃顆粒聯(lián)合西米替丁治療兒童IgA血管炎具有較好的療效，縮短癥狀消失時(shí)間，調(diào)節(jié)患兒的免疫狀態(tài)，值得臨床進(jìn)行推廣。

Objective To investigate the clinical efficacy of Huaiqihuang Granules combined with cimetidine in treatment of IgA vasculitis in children. Methods A total of 120 children with IgA vasculitis admitted to Chengdu Children’s Specialized Hospital from January 1, 2020 to December 31, 2022 were selected. All children were divided into control group (60 cases) and treatment group (60 cases) according to the computer random arrangement method. Patients in control group received intravenous infusion of Cimetidine Injection at a dose of 5 mg/kg each time, twice daily. Patients in treatment group took Huaiqi Huang Granules dissolved in warm water on basis of control group, 1 bag once, twice daily. The two groups of children were treated continuously for two weeks respectively. The clinical efficacy and symptom disappearance time of two groups were observed, and the changes of renal function, biochemical indicators and serum immune indicators before and after treatment in two groups were compared. Results The total effective rate of the treatment group (93.33%) was higher than that of the control group (80.00%, P < 0.05). After treatment, the disappearance time of skin purpura, joint pain and abdominal pain in treatment group was significantly shorter than that in control group (P < 0.05). After treatment, the levels of Scr, 24 h urine protein quantity and urine sediment red blood cell count in both groups were lower than those before treatment (P < 0.05). The levels of Scr, 24 h urine protein quantity and urine sediment red blood cell count of the children in treatment group were lower than those in control group (P < 0.05). After treatment, the IgA levels in both groups were lower than those before treatment, but IgM and IgG were higher than those before treatment (P < 0.05). The IgA level of children in treatment group was lower than that in control group, but IgM and IgG levels were higher than those in control group (P < 0.05). Conclusion The combination of Huaiqi Huang Granules and cimetidine in treatment of IgA vasculitis in children has a good therapeutic effect, shortens the disappearance time of symptoms, and regulates the immune status of children, which is worthy of clinical promotion.
