[摘要]
目的 探討血必凈輔助治療重癥肺炎的療效及對(duì)患者腎功能和血清Th1/Th2細(xì)胞因子的影響。方法 取2015年1月-2017年2月南陽(yáng)醫(yī)學(xué)高等專科學(xué)校第一附屬醫(yī)院診治的196例重癥肺炎患者,根據(jù)隨機(jī)數(shù)字表法分為對(duì)照組和觀察組各98例。均給予吸氧、化痰和營(yíng)養(yǎng)支持等常規(guī)指標(biāo)和抗生素治療;觀察組在此基礎(chǔ)上給予血必凈治療,連續(xù)治療1周。比較兩組的療效,治療前后的腎功能指標(biāo)水平以及血清Th1/Th2細(xì)胞因子水平。結(jié)果 觀察組的治療總有效率為94.90%,高于對(duì)照組的78.57%,差異有統(tǒng)計(jì)學(xué)意義(P < 0.05)。治療前,兩組的血肌酐(SCr)、血尿素氮(BUN)和內(nèi)生肌酐清除率(Ccr)水平差異無(wú)統(tǒng)計(jì)學(xué)意義;治療后,對(duì)照組的SCr、BUN水平略高于治療前,Ccr水平略低于治療前但不具統(tǒng)計(jì)學(xué)差異,而觀察組的SCr、BUN水平高于治療前,Ccr水平低于治療前,差異有統(tǒng)計(jì)學(xué)意義(P < 0.05);治療后觀察組的SCr、BUN水平高于對(duì)照組,且Ccr水平低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P < 0.05)。治療前,兩組的干擾素-γ(INF-γ)和白細(xì)胞介素-4(IL-4)水平和INF-γ/IL-4差異無(wú)統(tǒng)計(jì)學(xué)意義;治療后兩組的INF-γ水平、INF-γ/IL-4高于治療前,IL-4水平低于治療前,同組治療前后比較差異有統(tǒng)計(jì)學(xué)意義(P < 0.05);且治療后觀察組的INF-γ水平、INF-γ/IL-4高于對(duì)照組,IL-4水平低于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P < 0.05)。結(jié)論 血必凈聯(lián)合抗生素治療重癥肺炎效果明顯,有利于保護(hù)腎臟功能,調(diào)節(jié)血清Th1/Th2細(xì)胞因子水平,增強(qiáng)免疫力。

[Abstract]
Objective To discuss the effect of Xuebijing combined antibiotics on the efficacy, renal function and Th1/Th2 cytokines of severe pneumonia patients. Methods A total of 196 severe pneumonia patients treated in Nanyang Station South Road Medical Special Hospital from January 2015 to February 2017 were studied. All patients were treated with routine indicators such as oxygen absorption, phlegm and nutrition support and antibiotic therapy. On this basis, observation group was given Xuebijing therapy. The efficacy, renal function and Th1/Th2 cytokines before and after treatment were compared. Results The total effective rate of observation group was higher than control group (P < 0.05). Before treatment, the difference in SCr, BUN and Ccr levels of two groups was no statistically significant, the SCr, BUN levels of observation group were higher and Ccr level was lower than those of control group after treatment (P < 0.05), the SCr, BUN levels of observation group after treatment were higher and Ccr level was lower than those before treatment (P < 0.05), the SCr, BUN levels of control group after treatment were slightly higher and Ccr level was slightly lower than those before treatment (P < 0.05) but the difference showed no statistically significant. Before treatment, the difference in INF-γ, IL-4 levels and INF-γ/IL-4 of two groups showed no statistically significant, the INF-γlevel and INF-γ/IL-4 of two groups were higher and IL-4 level was lower than those before treatment (P < 0.05). After treatment, the INF-γlevel and INF-γ/IL-4 of observation group were higher and IL-4 level was lower than control group (P < 0.05). Conclusion The effective treatment of Xuebijing combined antibiotics is obvious, is beneficial to protectrenal function and regulate serum Th1/Th2 cytokines level and enhance immunity.
