[摘要]
目的 探討門冬胰島素聯(lián)合地特胰島素對(duì)新診斷2型糖尿?。═2DM)患者血糖和胰島β細(xì)胞功能的影響。方法 選取西電集團(tuán)醫(yī)院2017年1月-2018年12月收治的100例新診斷T2DM患者為研究對(duì)象,按就診順序分為兩組,每組50例。對(duì)照組采取地特胰島素皮下注射,觀察組采取門冬胰島素聯(lián)合地特胰島素皮下注射。對(duì)比兩組用藥對(duì)新診斷T2DM患者血糖和胰島β細(xì)胞功能的影響。結(jié)果 兩組治療前空腹血糖(FPG)、餐后2 h血糖(2 h PG)、糖化血紅蛋白(HbA1c)對(duì)比無(wú)統(tǒng)計(jì)學(xué)差異;經(jīng)治療,觀察組各項(xiàng)血糖指標(biāo)均明顯下降(P<0.05),下降幅度明顯優(yōu)于對(duì)照組(P<0.05)。兩組治療前空腹血清C肽(FC-P)、餐后2 h血清C肽(2 h C-P)對(duì)比無(wú)統(tǒng)計(jì)學(xué)差異;經(jīng)治療,觀察組的FC-P、2 h C-P明顯提高(P<0.05),且高于對(duì)照組,具有統(tǒng)計(jì)學(xué)差異(P<0.05);觀察組血糖達(dá)標(biāo)時(shí)間明顯短于對(duì)照組(P<0.05)。兩組治療前胰島β細(xì)胞功能指數(shù)(HBCI)對(duì)比無(wú)統(tǒng)計(jì)學(xué)差異,治療后,觀察組HBCI明顯高于對(duì)照組,具有統(tǒng)計(jì)學(xué)差異(P<0.05)。結(jié)論 對(duì)于新診斷T2DM患者,采取門冬胰島素聯(lián)合地特胰島素治療可有效控制血糖,促使胰島β細(xì)胞功能恢復(fù),治療效果顯著,值得臨床上進(jìn)一步推廣應(yīng)用。

Objective To investigate the effects of insulin aspartate combined with insulin detemir on blood glucose and islet β cell function in newly diagnosed T2DM patients. Methods A total of 100 newly diagnosed T2DM patients admitted to Xi'an Xidian Group Hospital from January 2017 to December 2018 were selected as study objects, and were divided into two groups according to the patients' medical order, 50 cases in each group. The control group received subcutaneous injection of insulin detemir, and the observation group received subcutaneous injection of insulin detemir combined with insulin aspart. The effect of blood glucose and island β cell function were compared. Results There was no significant difference in FPG, 2 h PG, and HbA1c between two groups before treatment. After treatment, all blood glucose indexes in two groups were significantly decreased, and the decrease rate in the observation group was significantly better than that in the control group (P<0.05). Serum C-peptide and 2 h C-P were not significantly different. After treatment, FC-P and 2 h C-P in the observation group were significantly higher than those in the control group (P<0.05), and the time of blood glucose attainment in the observation group was significantly shorter than that in the control group (P<0.05). There was no significant difference in HBCI before treatment. After treatment, HBCI in the observation group was significantly higher than that in the control group (P<0.05). Conclusion For newly diagnosed T2DM diabetes mellitus, insulin detemir combined with insulin aspartate can effectively control blood glucose, promote the recovery of pancreatic β cell function, and obtain satisfactory therapeutic effect, which is worthy of further clinical application.
