[摘要]
目的 探究對(duì)慢性阻塞性肺疾?。–OPD)繼發(fā)肺部真菌感染患者應(yīng)用小劑量氟康唑的臨床療效分析。方法 選取2016年2月-2018年2月信陽(yáng)市中心醫(yī)院收治的COPD繼發(fā)肺部感染患者共58例作為研究對(duì)象,隨機(jī)分為觀察組(29例)和對(duì)照組(29例)。對(duì)照組患者應(yīng)用廣譜抗菌藥物及解痙平喘、霧化吸入糖皮質(zhì)激素等治療;觀察組在對(duì)照組基礎(chǔ)上應(yīng)用小劑量氟康唑膠囊,100 mg/次,1次/d。兩組治療時(shí)間為2周。觀察比較兩組患者治療效果,真菌清除情況及不良反應(yīng)發(fā)生率。結(jié)果 治療后,觀察組總有效率為93.10%,顯著高于對(duì)照組的68.97%,差異具有統(tǒng)計(jì)學(xué)意義(P<0.05)。治療后,觀察組和對(duì)照組患者真菌清除率分別為96.55%和62.07%,觀察組真菌清除率明顯高于對(duì)照組,且差異具有統(tǒng)計(jì)學(xué)意義(P<0.05)。觀察組中出現(xiàn)1例胃腸道反應(yīng),而對(duì)照組中出現(xiàn)1例肝功能損害,1例皮疹,兩組患者的不良反應(yīng)發(fā)生情況無(wú)顯著差異。結(jié)論 小劑量氟康唑?qū)OPD繼發(fā)肺部真菌感染療效顯著,不良反應(yīng)較少,建議臨床推廣應(yīng)用。

[Abstract]
Objective To investigate the clinical efficacy of low-dose fluconazole in treatment of pulmonary fungal infection secondary to COPD. Methods A total of 58 cases of pulmonary fungal infection secondary to COPD were selected from February 2016 to February 2018 in Xinyang Central Hospital. All the patients were divided into observation (29 cases) and control (29 cases) groups. The control group were given broad-spectrum antibiotics, spasmodic antiasthmatic, atomizing inhaled corticosteroids, etc. The observation group was treated with low dose Fluconazole Capsules on the basis of the control group, 100 mg/time, once daily. Patients were treated for 2 weeks. The therapeutic effect was observed, and the fungal clearance and incidence of adverse reactions in two groups were compared. Results After treatment, the total effective rate was 93.10%, significantly higher than 68.97% of control group, and the difference was statistically significant (P<0.05). After treatment, the fungal clearance rate of the observation group was 96.55% while in the control group was 62.07%. The fungal clearance rate of the observation group was significantly higher than that of the control group, and the difference was significant (P<0.05). There was 1 case of gastrointestinal reaction in the observation group, 1 case of liver function damage and 1 case of rash in the control group. There was no significant difference in adverse reactions between the two groups. Conclusion Low dose fluconazole has significant curative effect on pulmonary fungal infections secondary to COPD with fewer adverse reactions, which is recommended to promote clinical application.
