[摘要]
目的 探討骨松寶顆粒聯(lián)合硫酸氨基葡萄糖治療膝骨關節(jié)炎的臨床療效。方法 納入2019年10月—2020年2月于南陽市第二人民醫(yī)院骨科門診及住院治療的86例膝骨關節(jié)炎患者作為研究對象,按照隨機數(shù)表法將患者分為對照組和觀察組,每組各43例。對照組患者口服硫酸氨基葡萄糖膠囊,2粒/次,3次/d。觀察組在對照組治療的基礎上口服骨松寶顆粒,1袋/次,3次/d。兩組各治療6周。觀察兩組患者的臨床療效,同時比較兩組治療前后的視覺模擬評分(VAS)、Lysholm評分、血清炎癥因子及骨代謝指標水平。結果 治療后,觀察組總有效率為95.34%,顯著高于對照組的79.07%,差異具有統(tǒng)計學意義(P<0.05)。治療后,兩組VAS評分顯著降低,而Lysholm評分顯著升高,同組治療前后比較差異具有統(tǒng)計學意義(P<0.05);治療后,觀察組VAS評分顯著低于對照組,而Lysholm評分高于對照組,差異均具有統(tǒng)計學意義(P<0.05)。治療后,兩組腫瘤壞死因子-α(TNF-α)、白細胞介素-1β(IL-1β)、白細胞介素-6(IL-6)水平較顯著降低(P<0.05),且治療后,觀察組TNF-α、IL-1β、IL-6水平顯著低于對照組,差異均具有統(tǒng)計學意義(P<0.05)。治療后,兩組抗酒石酸鹽酸性磷酸酶異構體5b(TRACP-5b)水平顯著降低,而骨鈣素(BGP)和骨特異性堿性磷酸酶(BALP)水平顯著升高,同組治療前后比較差異具有統(tǒng)計學意義(P<0.05);治療后,觀察組TRACP-5b水平顯著低于對照組,而BGP和BALP水平顯著高于對照組,兩組比較差異具有統(tǒng)計學意義(P<0.05)。結論 骨松寶顆粒聯(lián)合硫酸氨基葡萄糖治療膝骨關節(jié)炎的臨床療效較好且安全性高,能明顯改善膝關節(jié)疼痛癥狀及關節(jié)功能,可能機制與其能顯著降低機體炎癥因子水平及改善骨代謝有關。

[Abstract]
Objective To investigate the clinical effect of Gusongbao Granules combined with glucosamine sulfate in treatment of knee osteoarthritis. Methods A total of 86 patients with knee osteoarthritis in the department of orthopedics, The Second People's Hospital of Nanyang from October 2019 to February 2020 were included as the research objects. According to the random number table method, the patienthe control group were po administered with Glucosamine Sulfate Capsules, 2 capsules/time, 3 times daily. Patients in observation group were po administered with Gusongbao Granules on the basis of control group, 1 bag per time and 3 times daily. Both groups were treated for a total of 6 weeks. The clinical efficacy in two groups of patients was observed, and the VAS score, Lysholm score, serum inflammatory factors, and bone metabolism indexes of two groups before and after treatment were compared. Results After treatment, the total effective rate of the observation group was 95.34%, which was significantly higher than 79.07% of the control group, and the difference was statistically significant (P<0.05). After treatment, VAS scores of two groups were significantly reduced, while Lysholm scores were significantly increased, with statistically significant differences between two groups before and after treatment (P<0.05). After treatment, VAS score of the observation group was significantly lower than that of the control group, while Lysholm score was higher than that of the control group, with statistically significant differences (P<0.05). After treatment, the levels of TNF-α, IL-1β, and IL-6 in two groups were significantly decreased (P<0.05), and levels of TNF-α, IL-1β, and IL-6 in the observation group were significantly lower than those in the control group, the differences were statistically significant (P<0.05). After treatment, TRACP-5b levels in two groups were significantly decreased, while BGP and BALP levels were significantly increased, with statistically significant differences before and after treatment in the same group (P<0.05). After treatment, TRACP-5b levels in the observation group were significantly lower than those in the control group, while BGP and BALP levels were significantly higher than those in the control group, with statistically significant differences between the two groups (P<0.05). Conclusions Gusongbao Granules combined with glucosamine sulfate in treatment of knee osteoarthritis has good clinical efficacy and high safety, and can significantly improve the symptoms and joint function of the knee joint. The possible mechanism is related to its ability to significantly reduce the expression level of inflammatory factors in the body and improve bone metabolism.
