目的 系統(tǒng)評價戊乙奎醚預防剖宮產術應用卡前列素氨丁三醇所引發(fā)的不良反應。方法 計算機檢索PubMed、Embase、Cochrane圖書館、中國生物醫(yī)學文獻數(shù)據庫（CBM）、中國學術期刊全文數(shù)據庫（CNKI）、維普中文期刊全文數(shù)據庫（VIP）和萬方數(shù)據庫，檢索時限均為建庫起至2020年5月，收集戊乙奎醚降低卡前列素氨丁三醇在剖宮產術中不良反應的隨機對照研究（RCT），采用RevMan 5.3統(tǒng)計軟件對惡心、嘔吐、面部潮紅、胸悶氣短、心悸、腹痛及血壓升高等不良反應發(fā)生率進行Meta分析。結果 共納入13個RCTs，1 027例患者。Meta分析結果顯示：戊乙奎醚組惡心發(fā)生率[RR=0.36，95% CI=0.29~0.45，P<0.01]、嘔吐發(fā)生率[RR=0.21，95% CI=0.15~0.30，P<0.01]、面部潮紅發(fā)生率[RR=0.37，95% CI=0.26~0.52，P<0.01]、胸悶氣短發(fā)生率[RR=0.32，95% CI=0.26~0.40，P<0.01]、心悸發(fā)生率[RR=0.38，95% CI=0.28~0.50，P<0.01]、腹痛發(fā)生率[RR=0.52，95% CI=0.35~0.76，P<0.01]和血壓升高發(fā)生率[RR=0.43，95% CI=0.22~0.85，P=0.02]均顯著低于對照組。結論 戊乙奎醚能顯著降低卡前列素氨丁三醇致惡心、嘔吐、面部潮紅、胸悶氣短、心悸、腹痛及血壓升高等不良反應的發(fā)生。

Objective To systematically evaluate penehyclidine to reduce the adverse effects of carprostamine trometamol during cesarean section. Methods Retrieved from PubMed, Embase, Cochrane library, CBM, CNKI, VIP and Wanfang database, randomized controlled trials (RCTs) about the penehyclidine to reduce the adverse effects of carprostamine trometamol during cesarean section were collected during database establishment to May 2020. Meta-analysis was performed for the incidence of nausea, vomiting, facial flushing, chest tightness, shortness of breath, palpitations, abdominal pain and increased blood pressure by using Rev Man 5.3 statistical software. Results A total of 13 RCTs were included, involving 1 027 patients. Meta-analysis showed that the penehyclidine group in the incidence of nausea in[RR=0.36, 95%CI=0.29 to 0.45, P<0.01], the incidence of vomiting[RR=0.21, 95%CI=0.15 to 0.30, P<0.01], the incidence of facial flushing[RR=0.37, 95%CI=0.26 to 0.52, P<0.01], incidence of chest tightness[RR=0.32, 95%CI=0.26 to 0.40, P<0.01], incidence of heart palpitations[RR=0.38, 95%CI=0.28 to 0.50, P<0.01], incidence of abdominal pain[RR=0.52, 95%CI=0.35 to 0.76, P<0.01] and incidence of high blood pressure[RR=0.43, 95%CI=0.22 to 0.85, P=0.02] significantly lower than the control group. Conclusion The results of this study shows that penehyclidine can significantly reduce the incidence of nausea, vomiting, facial flushing, chest tightness, palpitations, abdominal pain and increased blood pressure caused by carprostamine trometamol.
