[摘要]
肺癌在我國癌癥中發(fā)病率和死亡率均居首位。當(dāng)前肺癌的治療仍以手術(shù)、放療、化療及靶向治療等為主,但此類療法不僅適用范圍有限,還可能引發(fā)多種不良反應(yīng),進而影響患者生活質(zhì)量及預(yù)后。中藥在肺癌治療領(lǐng)域積累了豐富的理論與實踐經(jīng)驗,兼具不良反應(yīng)少、多靶點作用等優(yōu)勢。近年來大量研究表明,中藥皂苷類成分可通過誘導(dǎo)細胞凋亡、抑制增殖與侵襲、逆轉(zhuǎn)耐藥、調(diào)控上皮間質(zhì)轉(zhuǎn)化(EMT)、調(diào)節(jié)腫瘤免疫微環(huán)境、干預(yù)糖酵解、抑制血管生成、誘導(dǎo)鐵死亡及調(diào)控自噬等多種機制,延緩肺癌發(fā)生發(fā)展。系統(tǒng)總結(jié)中藥皂苷類成分治療肺癌的作用機制研究成果,以期為肺癌新藥研發(fā)提供思路、為中藥抗肺癌研究提供理論支持,并為后續(xù)深入探索奠定基礎(chǔ)。

Lung cancer has the highest incidence and mortality rate among all cancers in China. Currently, the treatment of lung cancer mainly relies on surgery, radiotherapy, chemotherapy and targeted therapy, but these methods have limited application scope and may cause various adverse reactions, which can affect the quality of life and prognosis of patients. Traditional Chinese medicine has accumulated rich theoretical and practical experience in the treatment of lung cancer, and has the advantages of fewer adverse reactions and multi-target effects. In recent years, a large number of studies have shown that saponin components of traditional Chinese medicine can delay the occurrence and development of lung cancer through multiple mechanisms, such as inducing apoptosis, inhibiting proliferation and invasion, reversing drug resistance, regulating epithelial-mesenchymal transition (EMT), modulating the tumor immune microenvironment, interfering with glycolysis, inhibiting angiogenesis, inducing ferroptosis and regulating autophagy. Systematically summarizing the research results on the mechanism of action of saponin components of traditional Chinese medicine in the treatment of lung cancer is expected to provide ideas for the development of new drugs for lung cancer, theoretical support for the research of traditional Chinese medicine against lung cancer, and lay the foundation for further in-depth exploration.
