[摘要]
目的 研究丙種球蛋白聯(lián)合布地奈德對新生兒肺炎心肌酶譜、血清炎癥介質(zhì)和肺功能的影響。方法 選擇2015年1月—2017年12月西電集團醫(yī)院收治的80例新生兒肺炎患者,隨機分為兩組。對照組單純霧化吸入布地奈德治療,觀察組聯(lián)合靜脈滴注丙種球蛋白。兩組均治療7 d。比較兩組的治療效果,治療前后的心肌酶譜、血清炎癥介質(zhì)和肺功能。結(jié)果 觀察組的有效率明顯高于對照組,差異有統(tǒng)計學(xué)意義(P<0.05)。兩組治療后的血清肌酸激酶(CK)、乳酸脫氫酶(LDH)、肌酸激酶同工酶(CK-MB)水平均明顯降低,同組治療前后比較差異有統(tǒng)計學(xué)意義(P<0.05);且觀察組明顯低于對照組,差異有統(tǒng)計學(xué)意義(P<0.05)。兩組治療后的血清C反應(yīng)蛋白(CRP)、白細胞介素(IL)-4、IL-6水平均明顯降低,IL-10水平均明顯升高,同組治療前后比較差異均有統(tǒng)計學(xué)意義(P<0.05);且觀察組明顯優(yōu)于對照組,組間差異有統(tǒng)計學(xué)意義(P<0.05)。兩組治療后的第一秒最大呼氣量(FEV1)、用力肺活量(FVC)、最大呼氣流速峰值(PEF)均明顯升高,同組治療前后比較差異有統(tǒng)計學(xué)意義(P<0.05);且觀察組明顯高于對照組,差異有統(tǒng)計學(xué)意義(P<0.05)。結(jié)論 丙種球蛋白聯(lián)合布地奈德可以有效改善新生兒肺炎的心肌酶譜、血清炎性介質(zhì)和肺功能,提高治療效果。

[Abstract]
Objective To investigate the effect of budesonide combined with gamma globulin on myocardial enzymes, serum inflammatory mediators and lung function in neonates with pneumonia. Methods Selected 80 cases of neonates with pneumonia who were treated in our hospital from January 2015 to December 2017, divided into two groups randomly. The control group was inhaled budesonide, the observation group was treated with intravenous immunoglobulin. The therapeutic effects, myocardial enzymes, serum inflammatory mediators and lung function were compared.Results The effective rate of the observation group was significantly higher than control group (P<0.05); the serum levels of CK, LDH and CK-MB in the two groups were significantly decreased (P<0.05), and the observation group was more significant (P<0.05); the average levels of serum CRP, IL-4 and IL-6 in the two groups were significantly decreased (P<0.05), and the level of IL-10 increased significantly (P<0.05), and the observation group was more significant (P<0.05); the FEV1, FVC and PEF of the two groups increased significantly after treatment (P<0.05), and the observation group was more obvious (P<0.05). Conclusion Budesonide combined with gamma globulin can effectively improve the myocardial enzymes, serum inflammatory mediators and lung function in neonates with pneumonia, and improve the therapeutic effect.
